The last-16 tie went straight to penalties after the 90 minutes, as per Italian Cup rules.
Cagliari's fourth penalty taker, Mattia Felici, was the first to blink, his attempt crashing against the crossbar.
But Brazilian David Neres missed his chance to be the match-winner as his next effort was easily saved by Elia Caprile.
Napoli's goalkeeper, Vanja Milinkovic-Savic, made a crucial save in the penalty shootout, denying Zito Luvumbo's attempt, and Alessandro Buongiorno scored the winning penalty, securing Napoli's spot in the quarterfinals. They'll face the winner of the Como vs. Fiorentina match, scheduled for late January.
Inter Milan dominated Venezia, winning 5-1 at the San Siro, with Marcus Thuram scoring a brace. Meanwhile, Napoli edged out Cagliari in the shoot-out, and the round of 16 continues on Thursday with Bologna hosting Parma.
