Williams could manage just 10 minutes in Thursday’s 118-91 blowout loss and didn’t return for the fourth quarter.
San Antonio’s win tied the best-of-seven series 3-3, setting up a decisive Game 7 in Oklahoma City on Saturday.
Williams was a key piece in the Thunder’s championship run last season.
Saturday’s winner will face the New York Knicks for the title.
The Knicks come in hot and rested after sweeping Cleveland and Philadelphia, riding an 11-game win streak into the Finals.
